Shoreline Hospice Care treats health information under HIPAA. This website is not a patient portal. Do not send Social Security numbers, insurance ID cards, or detailed medical records through the contact form.

Inquiries submitted here are used only to return your call or email about hospice services. We do not sell this information. After you become a patient, a separate Notice of Privacy Practices applies to your clinical record.

Patient services are provided without regard to race, color, age, gender, religion, sexual orientation, handicap (mental or physical), or place of national origin.
